How do I report a change to my demographic information (i.e. residential address), tobacco status, or plan information?

You can report changes to your enrollment online, in-person with a certified Connect for Health Colorado Broker or Health Coverage Guide, or over-the-phone with a trained Service Representative. You can get in-person assistance by visiting the Connect For Health Colorado website.
