What is IRS Form 1095-B: Health Coverage?

Internal Revenue Service (IRS) Form 1095-B is a federal tax document that includes information about the months in the previous year that you and/or others in your household were enrolled in qualifying health care coverage, also known as Minimum Essential Coverage (MEC).

IRS 1095-B forms sent out by the Colorado Department of Health Care Policy and Financing will show the months that you were enrolled in Health First Colorado (Colorado’s Medicaid Program) or Child Health Plan Plus (CHP+).
