Why haven’t I received my 1095-B Form in the mail filing-cab
You don’t need to provide proof of health insurance coverage in order to file your taxes. Therefore, Health First Colorado (Colorado’s Medicaid program) will not be mailing out 1095-B forms this year.
I want a copy of my 1095-B form. computerPEAK is the fastest way to obtain a copy of the 1095-B Form. Go to the Mail Center in your Colorado.gov/PEAK account. There are copies of your form(s) and cover letter there. If you are a new PEAK user, create an account and visit the mail center. Click on “communications” and you will be able to download a copy of your 1095-B Form.
